Shift Timings: 6 AM - 6 :30 PM

Position Summary:
Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides high-quality nursing care, ensuring positive clinical outcomes and patient/family satisfaction. The LPN practices within the scope defined by the State Nurse Practice Act, contributing to nursing assessments, care planning, direct patient care, and supervision of unlicensed staff.
Responsibilities & Accountabilities:
- Patient Evaluation & Care Planning: Assists in nursing assessments and contributes to individualized care plans.
- Direct Patient Care: Provides hands-on nursing care while ensuring patient comfort and safety.
- Supervision of Unlicensed Staff: Monitors and oversees patient care provided by unlicensed personnel.
- Medication Management: Conducts drug counts at the beginning and end of shifts, as well as when necessary.
- Physician Assistance: Supports physicians during rounds and patient examinations as needed.
- Communication & Coordination: Participates in shift-to-shift handoffs and collaborates with other departments for effective patient care.
- Professional Development: Attends all required in-service training sessions to enhance nursing practice.
- Orientation & Training: Assists in the onboarding and training of newly hired nursing staff.
- Flexibility & Commitment: Willing to stay beyond scheduled shifts when necessary to meet staffing requirements and patient needs.
- Additional Duties: Performs other RN-assigned tasks as required.
Educational & Vocational Requirements:
- Graduate of an accredited practical or vocational nursing program.
- Active state licensure as an LPN.
- Preferred experience in rehabilitative or geriatric nursing.
- Current BLS/CPR certification (required for SNFs, Behavioral Centers, and applicable ALFs/ILFs per state regulations)
