Job Title: Home Health Aide (Floating Position)
Location: New York State
Schedule: Saturday, Sunday, Monday, Thursday, Friday (Shift times vary: 8 AM–4 PM, 4 PM–12 AM, 12 AM–8 AM)
Job Summary:
We are seeking a dedicated Home Health Aide (HHA) to provide compassionate care to multiple participants in accordance with individualized care plans. This floating position requires flexibility in shift scheduling and the ability to adapt to varying patient needs.
Key Responsibilities:
- Assist patients with daily living activities, including personal hygiene, dressing, and meal preparation.
- Provide companionship and emotional support to enhance patient well-being.
- Follow prescribed care plans and work under the supervision of a registered nurse.
- Monitor and report any changes in patient health or behavior.
- Maintain a clean and safe living environment for patients.
- Assist with light housekeeping and transportation as needed.
- Adhere to all safety, infection control, and compliance regulations.
Qualifications & Requirements:
- Active Home Health Aide (HHA) Certification (Valid in New York State).
- Basic Life Support (BLS) Certification required.
- Flexibility to work a rotating schedule, including weekends and overnight shifts.
- Strong interpersonal skills and a compassionate approach to patient care.
- Ability to work independently and follow detailed care instructions.
Certification Maintenance:
- HHAs in New York State must work for a certified/licensed home health agency to maintain certification.
- The employing agency provides nursing supervision and requires 12 hours of annual in-service training.
- If employment with a home health agency is discontinued, certification lapses two years after the last work date.
