Social Worker – Long Island, NY
Job Type: Full-Time
Schedule: 35 hrs/week (some nights & weekends required)
Salary: $58,000–$60,000 annually
About the Role: As a Social Worker, you'll provide counseling, and crisis intervention, and coordinate care plans to support successful family reunification or independent living. You'll work closely with families, schools, and service providers to ensure comprehensive, compassionate care.
Key Responsibilities:
- Provide individual/group counseling & crisis support
- Develop and manage treatment plans
- Facilitate family engagement and discharge planning
- Maintain Medicaid-compliant documentation
- Collaborate with schools and outside providers
- Lead team meetings and support staff development
Qualifications:
- Master’s in Social Work (LMSW/LCSW preferred)
- 2 years’ experience with adolescents or in residential settings
- Strong documentation and MS Office skills
- Valid NYS driver’s license
