Mon-Fri 8a-4:30p; Includes weekend on-call rotation

Purpose of Position
This position will coordinate and oversee the care and service of patients to achieve desired clinical and resource outcomes. Other duties include providing education and supporting patients and families related to their medical condition.
Job Requirements
- Associates Degree Nurse or Bachelors of Science Nursing Degree
- Current licensure with the Texas Board of Nurse Examiners
- Two years of experience in acute Medical/Surgical facility is required
- Two years of experience in Case Management is preferred
- Meditech Expanse knowledge is preferred
- Obtains and maintains American Heart Association BLS for Healthcare Provider
