Position Summary:
The Evening Nurse Supervisor is responsible for the operation of nursing operations facility-wide, including ensuring quality of management of resident care and assigned staff. The Evening Nurse Supervisor will ensure that services are provided with a resident care focus, in accordance with the nursing process, current standards of practice, and state and federal regulations. Additionally, all services are provided in accordance with the mission of Guardian Angels.
Job Duties:
- Responsible for planning, organizing and directing resident-centered, quality driven care on assigned nursing stations.
- Ensures accurate and timely performance of resident assessments.
- Manages day-to-day activities on the nursing unit to provide the highest quality of care for residents served.
- Reviews information from PCC dashboard, progress notes, assessments, incident reports, flow sheets, and care plans to keep abreast of changes in resident condition.
- Ensures assessments are completed timely and accurately. Assists in conducting assessments where needed.
- Provides care and services in accordance with standard of practice to avoid decline in resident condition that is not clinically unavoidable.
- Monitors indicators of potential care-related concerns, such as falls, weight loss, pressure ulcers, declines in ROM and mobility, continence concerns, etc., and implements interventions in a timely manner to decrease the risk of concerns.
- Ensures timely physician, family notification of changes in resident condition, e.g., falls, weight loss, pressure ulcers, etc.
- Monitors, audits, and assists with the destruction of medications.
- Performs routine audits of med carts and med rooms as designated.
- Ensures all orders have been processed by the end of the day with no outstanding in a queue or needing section check.
- Processes new admit orders and double checks new admit orders for accuracy and competition.
- Ensures documentation complies with standard of practice and effectively describes resident’s condition. Updates electronic plan of care timely as changes occur in resident condition.
- Ensures staff complete assigned responsibilities based on order sets for documentation needs.
- Ensures continuity between shifts, days of week; follow through on resident needs.
- Handles staffing issues, including call ins, floating staff, making calls for replacements etc.
- Ensures unit staff are aware of QAPI projects and the individual’s role in the project.
- Meets with team to problem solve, communicate changes, enhance teamwork.
- Promptly reports suspected abuse or neglect to Director of Nursing. Participates in vulnerable adult investigations with SW and DON, including interviewing staff, residents, collecting written statements.
- Provides staff support during annual MDH survey or OHFC investigations.
- Keeps Director of Nursing involved in rounding, provides report at end of shift.
- Makes frequent rounds, monitors resident: resident and staff interactions to ensure safe environment.
- Follows the manufacturer’s instructions for use of all equipment, products and chemicals.
- Ensures staff utilize appropriate lifting devices and good body mechanics throughout the day to avoid injury.
- Provides staff recognition, coaching, counseling and discipline as appropriate. Participates in the hiring and termination process.
- Conducts performance appraisals on licensed staff annually (90-day review for new hires).
- Acts as a resource person and mentor for staff on unit.
- Actively participates in staff development and retention initiatives.
- Assists with staffing as needed in the absence of Staffing Coordinator.
- Provides on-call coverage for nursing staff in rotation with other Nurse Unit Managers.
- Rounds with staff to gain input into unit and client needs
- Creates and maintains a dignified, respectful, resident-centered environment for residents.
- Follows universal precautions, facility infection control guidelines, sanitation and safety standards of practice in all work and activities.
- Monitors for adherence to infection control standards including hand washing, appropriate use of PPE.
- Identifies and reports infections to infection preventionist. Promptly implements transmission-based precautions as indicated.
- All duties will be performed in accordance with current standards, guidelines, state and federal regulations.
- Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA.

- Maintains flexibility in work hours to meet the needs of the assigned unit. Meets with night and evening shift, overlaps shifts as necessary to meet needs of unit.
- Performs all other duties as assigned.
Qualifications:
- Five plus years prior RN experience preferred with at least one year in geriatrics.
- Three to five years prior supervisory experience preferred.
- Currently licensed as a Registered Nurse in the state of Minnesota; license is unrestricted.
- Willingness to work with other disciplines in coordinating and monitoring resident services and activities directed toward achieving the highest practicable level of physical, mental, psychosocial, and spiritual health of our residents.
- Point Click Care (PCC) experience required.
- Ability to develop/lead nursing team.
- Ability to work autonomously or collaboratively as situation dictates.
- Ability to varying working hours to meet needs of assigned unit. Work hours will primarily be day shift, however must be able to meet with, assess evening and night shift concerns as situation dictates. Maintains good attendance and punctuality.
- Excellent verbal and written (read, write) communication skills.
- Self-motivated, ability to prioritize.
- Time management skills required.
- Ability to be on-call or be manager on duty.
