Job Overview:
As a Personal Care Service Provider (PCSP), you will be responsible for assisting individuals with their daily personal needs, ensuring their comfort, safety, and well-being. This role involves assisting the client with non-medical daily living activities. Duties performed may include but are not limited to keeping house (making beds, doing laundry, washing dishes) and preparing meals and providing personal care. The PCSP functions under the direction, instruction and supervision of the RN assigned to the case
Duties:
· Assist the client with personal hygiene (bathing, shampoo, shaving and oral hygiene)
· Plans and prepares nutritious meals or modified diets as prescribed by a physician.
· Assists with activities of daily living which include dressing, ambulation, toileting, and safe transfer techniques as directed by the Plan of Care.
· Prompts client to take self-administered medications as per the care plan.
· Works with other community agencies involved with the client as directed by the Plan of Care.
· Assists with treatments/therapies as ordered by physician and approved and supervised by RN and Plan of Care.
· Carries out assignments as instructed by PCSP care plan and notifies RN when unable to do so. If RN is unavailable, notify the Director of Client Services.
· Reports changes in clients’ condition or home situation immediately to the office.
· Use of Mobile App for electronic record of visit, including activity, time and mileage
· Performs routine housekeeping duties as related to a safe and comfortable environment for the client as directed by the Plan of Care.
· Accompany the client to necessary appointments, only when authorized by the office.
· Respect the cultural and religious practices of the client and their families
· Conduct oneself in a manner that promotes safety
Skills:
- Experience as a personal care service provider or caregiver
-Successful completion of PCSP training program that meets State regulations
- Ability to lift and transfer clients safely
- Knowledge of dementia care practices
- Strong interpersonal skills for effective communication and relationship-building
- Capability to provide direct support and assistance tailored to individual needs
