Job Summary
We are seeking a compassionate and dedicated Registered Nurse/Licensed Practical Nurse - Home Health to provide high-quality patient care in a home setting. Responsibilities
- Conduct thorough assessments of patients' health status and develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Monitor patients' progress and adjust care plans as necessary, documenting all changes accurately.
- Provide education and support to patients and their families regarding health management and home care practices.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
- Maintain compliance with all relevant healthcare regulations, including workers' compensation law.
- Offer support for patients requiring specialized care.
- Ensure the safety and comfort of patients during home visits, utilizing effective communication skills.
Qualifications
- Current Registered Nurse (RN)/Licensed Practical Nurse (LPN) license in the state of practice.
- Experience in hospital settings or home community care preferred.
- Knowledge of anatomy and patient care best practices is essential.
- Familiarity with systems related to workers' compensation law is a plus.
- Previous experience working with individuals with developmental disabilities or providing infant care is highly desirable.
- Strong interpersonal skills to build rapport with patients and their families.
- Ability to work independently while demonstrating sound clinical judgment.
- Excellent organizational skills and attention to detail.
