What are the responsibilities and job description for the Direct Service Worker (DSW) Supervisor position at Guidance Personal Care?

Responsibilities of DSW Supervisor
The responsibilities of the DSW supervisor include, but are not limited to:
- Supervise and monitor DSWs, assuring the company's operations run efficiently and effectively.
- Determine whether clients are receiving the services that are written in their plan of care
- Verify that the DSW is actually reporting to the home according to the frequency ordered in the plan of care
- Provide on-call support to staff during emergencies and locate staff or complete shifts to fill shifts when necessary
- Determine member’s satisfaction with the services member is receiving
- Attend all required trainings and meetings
- Must follow and adhere to all rules and procedures of GPC regarding confidentiality, documentation, and storage of client office & home records
- Conduct in-person supervisory visit with each DSW within 60 days of being hired or contracted and at least annually thereafter and documented
- Making in-home visits frequently or as needed if dictated by the individualized service plan (ISP) to address DSW performance, client change in status, or to assure services are provided effectively and in accordance with agency/state laws, and regulations, and ISP.
- Conduct unannounced visits to evaluate the abilities of DSW performing assigned duties, if goals are being met, if services are being provided in accordance with the ISP
- Document DSW/client relationship, services provided, provide instructions/comments to DSW during onsite visits and conduct client satisfaction with services delivered
- Visit the home of each client on a quarterly basis to determine whether the individual service plan is adequate, continues to need the services, and if the service plan needs revision
- Ensure accidents and incidents are reported and handled within state guidelines
