Overview:
Guided Journeys Outreach Services (GJOS) is seeking compassionate and dedicated QMHP-CS/Case Managers to provide Mental Health Targeted Case Management (MHTCM) and Skills Training services to children, adolescents, and adults in the community. We are looking for individuals who are passionate about client progress, organized in their documentation, and committed to delivering person-centered care.
Qualifications:
Must meet and maintain QMHP-CS definition per TMHP regarding education, experience and continuing education; must have working knowledge of the areas of need for youth who are at risk for out of home placement due to mental or behavioral instability; must have appropriate clinical skills to facilitate training and coping skills building for youth who are at risk for out of home placement and their families; must be able to work from a person-centered framework when providing clinical services; must have clinical interviewing skills; must have understanding or ability to train in appropriate clinical interventions; must be able to identify clinical risk factors for suicide, violence and other risky behaviors. A valid Driver’s License is also required for employment.
Key Responsibilities:
- Provide Targeted Case Management and Skills Training & Development based on each client’s Individual Support Plan (ISP)
- Conduct face-to-face visits at the client’s home, school, or community settings
- Develop and maintain individualized service plans and safety plans
- Coordinate services with families, schools, medical professionals, and community resources
- Ensure all documentation is completed within required timelines using EHR
- Attend supervision, training, and staff meetings as scheduled
- Maintain compliance with all; The MCO’s, The Agency, State, and Medicaid guidelines
- Ensure that service documentation is individualized based upon the needs and progress of the individual
- Submit all documentation by agency deadlines (within 24-48 hours after contact with client).
Qualifications:
- Bachelor’s degree in social work, psychology, counseling, or a related field
- Must meet QMHP-CS criteria per Texas Administrative Code
- Previous experience with mental health case management or skills training preferred
- Strong organizational and time management skills
- Valid Drivers License
- Reliable transportation and ability to travel within the service area
- Bilingual (Spanish) is a plus
