Job Overview
We are seeking a Case Manager for our rapidly growing non-medical home care agency. The ideal candidate will have a mix of sales and experience managing people. This is NOT a nursing case manager role.
Responsibilities
- Conduct thorough patient intakes to determine individual needs and develop personalized care plans.
- Recruit caregivers and assist in the hiring process
- Market to potential private pay and clients on the medicaid homecare waiver program
- Accurately complete and document patients needs to share with caregivers
Requirements
- Comfortable initiating conversations with strangers
- Strong organizational skills
- Ability to manage employees in a challenging enviroment
