Job Overview
The Service Coordinator assists the prospective participant to become a waiver participant and coordinates and monitors the provision of all services in the Service Plan. Services may include Medicaid State Plan services, non-Medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant’s independence, productivity and integration into the community while maintaining the health and welfare of the individual.
· Meet with potential participants.
· Assists the prospective participant in becoming a waiver participant and coordinates and monitors the provision of all services in the service plan. obtaining and filing an original Rights and Responsibilities sheet for each participant (for each service).
· Responsible for assuring that the Service Plan is implemented appropriately and supporting the participant to become an effective self-advocate and problem solver.
· Together with participant develop and implement the Service Plan, which reflects the participant’s goals.
Qualifications
· Active driver’s license
· Licensed Clinical Social Worker, Licensed Master Social Worker, Person with a Master’s or Doctorate in Psychology or Social Work, Certified Teacher of Students with Disabilities, Certified Rehabilitation Counselor, Licensed Physical or Occupational Therapist, Licensed Speech Pathologist or Registered Professional Nurse
· And minimum of one year of experience with case management and service coordination, information and referral services or service brokering of community bases serves for individuals with disabilities and/or TBI
OR
· Individual with Bachelor’s degree and 3 years of experience providing case management/service coordination information and referral services or service brokering of community based services for individuals with disabilities and/or TBI.
Must have intermediate level computer knowledge.
The ability to create and send emails to groups of individuals with attachments.
Must be organized.
Must know how to utilize the following Microsoft programs:
Word, Excel, Power Point and Outlook. The computer is 70% of the job.
