What's the Gig?
As our Rockstar Case Manager, you'll be the MVP coordinating seamless care between our amazing seniors, clinical teams, account managers, and facility partners. Your mission? Ensuring our beloved seniors experience top-tier mental health support every step of the way, from onboarding to ongoing care. This role is made for someone who's detail-oriented, relationship-focused, and ready to champion seniors' wellbeing!
Here's What You'll Crush:
Master of Communication:
- Be the go-to liaison linking nursing homes, assisted living facilities, and our clinical rockstars.
- Keep everyone in harmony through clear, timely, and proactive communication.
Documentation Dynamo:
- Secure essential paperwork (insurance face sheets, PoA records, etc.) and maintain meticulous records—because details matter!
- Collaborate to complete the Patient Readiness Form, ensuring we're fully prepared to give seniors the care they deserve.
- Stay ahead of compliance and regulatory requirements without breaking a sweat.
Facility Ambassador:
- Regularly visit your assigned facilities (3x per week minimum) to stay in tune with census data, identify new residents, and track status changes.
- Keep everyone looped in on admissions and transitions—because smooth care starts with you!
Senior Care Advocate:
- Own the senior patient journey—advocating fiercely for their mental health and quality of life.
- Partner with internal teams and account managers to proactively address concerns and optimize the delivery of exceptional care.
Are You Our Person?
- You’re a Licensed Clinical Social Worker (LCSW) with a passion for seniors.
- You have hands-on experience in mental health case management, ideally within senior living or nursing home settings.
