Purpose
The Integration Specialist provides the administrative nursing component that is integral to our holistic Behavioral Health Home (BHH) program. This is a unique role with our BHH Team where you will be utilizing your community-based nursing skills to assess, educate and wellness plan with program participants. The goal of this team is to engage individuals, promote wellness, and improve management of health conditions including mental illness, substance use disorders and chronic physical health conditions along with addressing the social determinants of health. This role is supervised by the Program Supervisor and works alongside a dynamic team of professionals.

The salary range for this position is between $60,000 to $65,000 annually. The range reflects the minimum and maximum salary for the position. Compensation will depend on the candidate’s professional experience, skills, and relevant education, training, and certifications.

Key Responsibilities
Duties and Responsibilities:
- Collaborating with clients and staff on the BHH Team.
- Utilize health knowledge to support individuals and their Health Action Plan.
- Assessment of individuals health and goal planning.
- Partnering with the Program Supervisor to provide wellness-based education and training to community-based workers.
- The ability to navigate and input electronic health records in a timely manner. Review and approval of documentation.
- Facilitate individual and group interventions in areas of health and medication management.
- Understanding of and respect for boundaries, values, and personal differences.
- Promote an inclusive, diverse, culturally competent, and respectful work environment.
- Navigating mental health crises with a person-centered approach.
- SMI and the functional impairments associated with co-occurring disorders.
- Referral process for psychiatry, primary care, or specialty clinics.
- Community based health resources
- Advanced verbal and written communication skills.
- Ability to self-motivate, manage competing priorites, and adhere to deadlines.
- Ability to adapt to changing conditions and be open to new processes.
- Exhibit a sense of personal and professional empowerment, to influence and create positive change motivate, manage competing priorities, and adhere to deadlines.
- Licensed by the state of Minnesota as a Licensed Practical Nurse.
- Experience in public health or other community-based nursing.
- Have one year of previous experience working within a mental health setting.
- Must be ambulatory and able to negotiate stairways.
- A valid Driver’s License, reliable vehicle, satisfactory driving record, and auto insurance are required.
- Must not be actively receiving services from Guild and cannot have received services within the last two years.
- This position is based at Guild St. Paul area; however, most of the work is in the field as services are provided to individuals wherever needed. This position has the potential to come into contact with bodily fluids, communicable diseases, tobacco smoke, and/or household pets.
