*** $2,000 hiring bonus - $1,000.00 after first 90 days of successful employment and $1000.00 after 9 months of employment ***
Guild is seeking a Certified Peer Support Specialist to join out Homeless Outreach team. This position provides direct support and services to individuals with serious mental illness and substance use disorder who are living in shelters, on the street, are without stable housing, or are at-risk of homelessness. The CPS will draw on their own experience with mental illness to provide peer counseling and direct support, as well as model wellness and assist individuals in their recovery.
As part of the team, will work in partnership and collaboration with social service and housing providers to ensure that individuals experiencing homelessness decrease the time between referral and being housed, make connections with other services as needed, and remain engaged.
Primary Responsibilities
- Connect with individuals referred to Guild via SMAC Coordinated Entry System.
- Locate individuals in the community and offer Guild services.
- Coordinate intake and perform an individualized needs assessment to develop a Housing Support Plan that addresses barriers to obtaining housing.
- Assist individuals in accessing services and resources by providing information, referrals, linkages, and advocacy.
- Connect individuals to suitable housing options like subsidized housing, Section 8, EHV, SROs, permanent supportive housing, affordable and market-rate housing, and other similar opportunities.
- Assist the individuals with the housing related paperwork, with prospective landlords, ensure completion of Coordinated Entry Assessment for potential priority housing opportunities.
- Provide crisis intervention to homeless individuals in the community or in a safe environment as needed.
- Partner with community and county partners to best serve individuals.
- Attend monthly trainings offered by DHS and stakeholders or as needed.
- Document all interactions and communications with individuals in the Homeless Management Information Systems and Guild’s EHR.
Qualifications
Level I Certified Peer Support Specialists must meet the following criteria:
- Qualified under Minnesota law as a Mental Health Rehabilitation Worker.
- Successfully completes the Minnesota Department of Human Services (DHS) approved Certified Peer Specialist training and certification exam.
- Level 1 Certified Peer Support Specialists: If you are yet to be certified or on the path to certification, we are able to consider your application.
Preferred
Level II Certified Peer Support Specialists must meet all requirements of a Level I CPS and the following criteria:
- Is qualified as a mental health practitioner per DHS requirements.
Additional Requirements
- Must be ambulatory and able to negotiate stairways.
- Valid Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
- Ability to perform CPR and First Aid and respond to emergencies.
- Must not be actively receiving services from Guild Incorporated and must not have received services within the last two years.
