Guild is hiring a Mental Health Case Manager to support our Youth Assertive Community Treatment (ACT) Teams. This position will serve clients in Ramsey, Dakota and Hennepin Counties. This position is responsible to assist individuals who have serious and persistent mental illnesses and / or chemical dependency to live self-directed and satisfying lives by providing services directly, minimally using outside resources and diverting hospitalizations by finding alternative interventions whenever possible. When there are other providers, there is close coordination and collaboration with these other outside resources to ensure the best possible service.
Primary Responsibilities
- Build trusting relationships with clients to encourage their active participation in treatment and recovery.
- Assist clients in managing their mental health symptoms, daily living skills, and overall well-being.
- Ensure timely medication delivery to clients and monitor adherence to prescribed treatment regimens.
- Assist clients in renewing or applying for benefits, including Medicaid, Social Security, housing assistance, and other community services.
- Collaborate with other service providers and natural supports, ensuring clear communication and coordination of services.
- Actively participate in team meetings and care conferences to discuss client progress and share insights with team members.
- Complete required documentation in compliance with organizational and regulatory standards.
- Perform other duties as assigned to support client care and department operations.
