WHAT YOU WILL DO:
The Padres de Crianza Community Navigator is a non-exempt position responsible for providing in-home support to children and Hispanic/Latino relative/non-relative caregivers of Hillsborough County, FL. The Community Navigator is responsible for providing linkage and referrals to services and supports, both formal and informal, appropriate to the familys needs as well as providing stress management education, parenting education and crisis intervention as needed.
- Complete client screening in a timely and effective manner - identify strengths, limitations, risk factors, and needs, and refer and coordinate the provision of concrete services to children and their families
- Complete intake, NCFAS-G R; and Family Support Plan with the caregiver and provide ongoing evaluation and monitoring of the Family Support Plan
- Complete termination session to ensure all Family Support Plan goals have been met
- Provide information, referral, and follow-up for appropriate community services, such as counseling, psychiatric services, financial, employment, parenting, housing, and other services as deemed appropriate to meet caregiver's needs
- Provide effective role modeling and mentoring to families in a family-centered, youth-guided approach congruent with the desired outcomes addressed in the Family Support Plan
- Maintain face-to-face and/or telephone contact with each assigned caregiver and with provider agencies servicing families to facilitate effective collaboration for appropriate services
- Work cooperatively with the Program Manager and Counselor on a weekly basis to ensure that the caregiver's needs are met
- Attend and participate in professional development training for professional growth, as required by the state of Florida and in accordance with agency policy and procedures
- Provide on-call services as assigned, abiding by program on-call policy and procedure
- Perform all other duties as assigned

WHAT YOU'LL NEED:
- High School diploma or equivalent
- Minimum one (1) years' experience working with children and families preferred.
- Bilingual required, English and Spanish
- ·Must possess knowledge of child development, including physical, mental, emotional, and cultural issues
- Knowledgeable of community resources, service referrals, and linkage
- Effective verbal and written communication skills
- Proficient in Microsoft Applications
- Time management skills
- Ability to work in the field at any given time
- Ability to work outside of regular working hours as needed
- Must have a valid drivers' license with auto insurance coverage
