This is a partially grant funded position
SUMMARY
This position will provide transportation to individuals with medical, mental health and/or substance abuse diagnoses. Transportation may be provided to meet basic needs and connect to community resources and/or to address treatment plan goals which may include psychosocial needs such as housing or workforce development.
ESSENTIAL QUALIFICATIONS
- Must be at least 18 years of age
- High School Diploma or General Education Degree (GED)
- Must possesses a valid driver’s license, reliable transportation and be insurable
- Able to demonstrate personal knowledge and experience with Native American populations
DUTIES AND RESPONSIBILITIES include, but are not limited to, the following:
- Transport patients to medical or behavioral health related appointments, to meet basic needs and connect to community resources and/or to address treatment plan goals which may include psychosocial needs such as housing or workforce development
- Actively work with patients to navigate and use community resources in an effective manner
- Documents all services provided in an electronic medical record accessible to all providers
- Communicate with interdisciplinary team regarding patient treatment plan and progress towards goals
- Assist in planning and implementing outreach/health promotion disease prevention activities

Assist citizens with the application/certification or re-certification for Tribal and outside programs
- These include but are not limited to Medicaid, Medicare, MI-Child, Healthy Kids, WIC, Commodities, energy assistance and housing needs
- Refer clients to other health/or human service resources, as needed
- Serve as a Goodwill Ambassador for the Tribe
- Develop and maintain working relationships with outside agencies/programs to access those resources
- Recognize and be sensitive to the cultural diversity inherent in the practice of self-care/health/wellness within Native American communities and educate appropriately
- Other duties as assigned may include assisting with grant activities, other program activities, and office tasks as needed to maintain a team approach to care delivery throughout HHS.

- Able to relate effectively with staff, clients and professional community
- Experience working with Native American populations
- Proficiency with basic computer applications
- Must have good organizational skills and the ability to multi-task
- Able to operate standard office equipment
- Must be patient, courteous and reliable and able to use sound judgment
- Flexible, highly organized and able to work independently
- Able to correspond with individuals throughout all levels of government and intergovernmental organizations
- Maintain the highest level of confidentiality of all matters
- Able to maintain an effective team working relationship with co-workers
- Data input/Electronic Health Record and competent/Recordkeeping
CONDITIONS OF EMPLOYMENT
- Will require non-traditional hours including working evenings, weekends and holidays to meet program needs and deadlines
- Must possess a valid driver’s license, reliable transportation, and be insurable
- Ability and willingness to travel for meetings and continuing education is required for this position
- CPR/First Aid Certification Required, may obtain after hire within first 6 months
- Mental Health First Aid Training Required, may obtain after hire within first 6 months
