Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences) and our independent living community (Fountaingate Gardens).
We currently have a career opportunity for a Nurse Manager, R.N. for a Rehab Unit who will exemplify the passion and commitment that is required to deliver the ultimate care for our residents in regard to doing admission assessments and all components for new admissions / re-admissions. The postion is Monday through Friday but will rotate holidays.
Responsibilities include, but are not limited to :
Assumes responsibilities for the formulation and implementation of patient care plans to meet the specialized needs of geriatric patients using nursing assessment and diagnostic skills.
Assures that nursing diagnosis for every resident is based on physical and psycho-social observation (self-care deficit needs).
Assures for every resident; changes in resident status and appropriate response by unit staff are continuous and timely.
Directs the application of knowledge in rehab / restorative nursing and liaison with Rehabilitation.
Develops and supervises nursing approaches for resident care (ie : nursing orders)
Assists in direction, supervision, instruction, and evaluation of other nursing personnel as assigned.
Observes, records, and reports patient data both verbally and in writing as dictated by departmental policy.
Participates in unit nursing and inter-disciplinary conferences. Assumes a role in planning unit management with Nursing Care Coordinator.
Assumes responsibility for adherence and support of the institutional and departmentalphilosophy and policy in professional practice.
Attends nursing and other institutional in-service programs and participates in departmental and institutional committees as assigned.
Performs related and miscellaneous duties as directed. In an emergency situation, may be expected to perform unrelated duties.
Ability to respond flexibly to a dynamic clinical environment.
Requirements include :
Graduate of an accredited professional R.N. nursing program with a current license to practice in New York State.
BSN preferred.
Current license required.
