What are the responsibilities and job description for the Care Coordinator position at Guthrie County Hospital?
Utilize assessment, planning, education, and evaluation to provide comprehensive patient care coordination. Assist patients in creating and implementing effective solutions to improve their health and wellness. Communicate effectively with patients, family members / caregivers, and other healthcare professionals to successfully execute patients’ plans of care. Participate in a multidisciplinary team as part of focusing on a patient centered medical home. Responsible for data analytics in the clinics, including a strong emphasis on report development, data integrity to assist with decision making, and working knowledge of EHR data structure to accurately and efficiently report findings in a meaningful manner.
Graduate from an accredited school/college of nursing- Current Registered Nurse or Licensed Practical Nurse license in the State of Iowa
- Basic Life Support (BLS)
- Bachelor of Science in Nursing (BSN) preferred
- Previous clinical experience in a medical office preferred
- Previous experience in Care Coordinator and Health Coach role preferred
