POSITION SUMMARY
Assists patients/residents in maintaining or improving their ability to manage their everyday physical, mental, psychosocial needs and accessing available services and social supports. This position involves meeting in the hospital, private residences, county offices or other public locations as needed. May include occasional work-related calls outside of business hours.
This is a full-time non-exempt position under the federal and state wage and hour laws, which means you are eligible for overtime pay after 40 hours in a work week. However, this position does allow flexible hours as occasional evening and weekend work may be required as job duties demand. Benefits include IPERS, medical, dental, vision, and life insurance. Paid vacation, sick, personal time, and comp time.
ESSENTIAL JOB DUTIES
The following duties are normal duties for this position. Other duties may be required and assigned
· Assesses patients admitted for discharge planning needs, coordinates discharge planning activities, and follows through with interventions.
· Maintains progress report for providers referring patients for social services.
· Makes arrangements for obtaining needed adaptive equipment, clothing, personal items, and other personal services as needed. Assists in arranging financial assistance through the appropriate agencies.
· Facilitates Mental Health transfers, alcohol/drug abuse treatment transfers, and abuse case reporting.
· Organizes and coordinates multidisciplinary discharge planning meetings for all patients as referred by providers.
· Collaborates on Crisis Intervention cases, provide training for staff, and be available for call-in consult as necessary.
· Finds options that meet the physical and emotional needs of referred patients.
· Coordinates between referred patient and family any medical needs, social needs, and discharge planning.
· Coordinates referred patient’s transitions from inpatient to outpatient rehabilitation, home health care programs or other facilities.
· Aides in the planning discharge of patients to other facilities.
· Maintains contact with family (with permission) to report changes on health current goals, discharge planning, and encouragement to participate in care planning.
· Works with certified entities on completing presumptive eligibility screening for Medicaid for referred clients.
· Gathers and evaluates data regarding available resources and participates in discharge planning.
· Provide or assists with staff and community education related to social services.
· Performs and maintains documentation.
· Implements infectious disease controls as appropriate including isolations procedures, proper use of hand washing and gloves.
· Follows HIPAA regulations and ensures that the confidentiality of patients’ medical, personal, and financial records; employee records; and organizational records is maintained.
· Participates in the organization’s continuous quality improvement (CQI) program by supporting quality improvement initiatives.
· Complies with federal and state regulatory requirements related to the performance of County Health services and hospital operations and requirements of the hospital compliance program.
· Presents self in a professional manner. Enhances professional growth and development through educational opportunities relevant to the job, as well as reading and/or contributing to current literature.
· Performs other duties as assigned.
QUALIFICATIONS AND EXPERIENCE
· Minimum of a BSW, RN or degree related to Human Services field including Sociology, Special Education, Rehabilitation Counseling, and Psychology.
· Dependent Adult & Child Abuse Mandatory Report training.
· Prior Care Coordinator experience preferred.
· Possession of a valid driver’s license. Must be able to obtain a State of Iowa driver’s license within 90 days of employment.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
· Ability to communicate effectively.
· Ability to work with minimal supervision.
· Ability to prioritize tasks and possess strong organizational skills, time management skills, and attention to detail.
· Ability to perform a variety of physical movements including standing, kneeling, pulling, pushing, and pulling/lifting objects under 50 lbs.
· Ability to operate Microsoft Office programs proficiently.
