Job Summary:
Serves as a Diabetes Care and Education Specialist (DCES) in the acute care environment.
Education:
- ASN is required.
- BSN is preferred
- Required RN licensure at time of hire.
- Certification as Diabetes Care and Education Specialist or able to sit for the exam and obtain certification within 12 months.
Experience:
- Two or more years of experience in the area of diabetes management is strongly preferred.
- Academic and/or experiential preparation in program management and in the care of people with chronic diseases.
- Demonstrated competence in teaching and in applying principles of teaching/learning process.
- Demonstrated competence in skills of communicating, leadership and creativity with groups.
- Demonstrated ability and/or willingness to develop skills in teaching and in applying principles of the teaching/learning process with adult learners and child learners.
- Experience in insulin pump therapy or willingness to learn all aspects of insulin pump therapy to become a certified pump trainer.
- Microsoft Office computer skills required; to include Word and Excel.
Essential Functions:
- Functions as part of a multidisciplinary team providing diabetes education and support for patients and families in the acute care setting.
- Uses the nursing process while providing individualized Diabetes Self-Management Education and Support (DSMES) in both one-to-one and group settings.
- Documents education delivery appropriately in the assigned computerized system.
- Responsible for maintaining yearly CEU requirements including staying up-to-date on Diabetes Standards of Care e.g., technology, treatment regimen, social determinants of health.
- Participates in quality improvement, compiling data, tracking outcomes and making recommendations for process improvement.
- Operates independently as well as collaboratively in carrying out the functions of the Diabetes Program and in the support of overall hospital goals.
