Guthrie Home Health New York (Vestal) is searching for a disciplined & compassionate per diem Registered Nurse Case Manager in the Home Health setting in the Southern Tier region of NY. Our amazing team of Case Managers are critical thinkers that help provide a high standard of care to patients in their homes. One year of Med/Surg, ER or ICU type experience is required.
The hours are typically 8:00 AM - 4:30 PM but the position offers flexibility with accountability. Being per diem you'll have flexibility to create your own schedule daily based on your patient load. The schedule does include on-call and weekend rotation.

Description:
Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
Education/License:
Diploma from an accredited school/college of nursing. Required professional licensure at time of hire.
Responsibilities:
Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.|
Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.|
Carries out physician and nursing orders. Attends team meetings and care plan meetings.|
Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.|
Participates in after hours/weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.
