What are the responsibilities and job description for the Social Worker - Full Time position at Guthrie Healthcare?
The schedule for this Social Worker position is 4 days are 8:30-5:00p and one shift is 10:30-7:00p per week in our Endicott office.
Guthrie Lourdes Mental Health is an Article 31 Office of Mental Health clinic serving patients from age 5 throughout life. This position involves diagnosing, treatment planning and psychotherapy. On site supervision is available by an LCSW as needed. Medication management is also provided through the clinic, and the clinic has multidisciplinary meetings weekly where clinical questions can be addressed or may be an educational meeting focusing on various mental health topics.
Job Summary:
Investigates, assesses and plans interventions to help patients cope with social, emotional, economic and environmental problems.
Education, License & Cert:
- LMSW, LCMS, LMHC, LMFC, other accepted licenses as outlined in OMH 599 regulations
Essential Functions:
- Provides psychosocial evaluation to meet patient's mental health related needs. Such services include, but are not limited to: Psychosocial Assessment, Risk Assessment, Treatment Planning Diagnosing, Crisis Intervention, collaborates with the team to provide appropriate communication for those requesting medication management, communicates with case manager and other collateral entities, and provides ethical treatment as outlined in OMH 599 regulations
