Guthrie Home Health New York (Vestal) is searching for a disciplined & compassionate Registered Nurse Case Manager in the Home Health setting in the Southern Tier region of NY. Our amazing team of Case Managers are critical thinkers that help provide a high standard of care to patients in their homes. One year of Med / Surg, ER or ICU type experience is required.The hours are typically 8 : 00 AM - 4 : 30 PM but the position offers flexibility with accountability. You'll have flexibility to create your own schedule daily based on your patient load. Required professional licensure at time of hire.Responsibilities : Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.|Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.|Carries out physician and nursing orders. Attends team meetings and care plan meetings.|Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.|Participates in after hours / weekend on-call rotation, as needed.
