Position Details :
- Status : Per Diem
- Schedule : Typically 8 : 00 AM - 4 : 30 PM, Monday - Friday, with flexibility to create your own schedule based on patient load. The Position does include a On-Call and Weekend Rotation

Description :
Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
Education / License :
Diploma from an accredited school / college of nursing. Required professional licensure at time of hire.
Responsibilities :
Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.|
Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.|
Carries out physician and nursing orders. Attends team meetings and care plan meetings.|
Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.|
Participates in after hours / weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.

Payrate : $37.50 to $53.06.
