Job Title: Care Manager
We are looking for a dynamic clinical care manager to join Gwen’s Girls clinical team. In this role, you'll be responsible for evaluating clients’ treatment plans, monitoring enrolled clients’ well-being, creating care progress reports, and ensuring clients are well informed regarding their care and treatment options.
Job Purpose: Clinical care managers oversee the quality of client care for clients enrolled in Gwen’s Girls clinical services. The duties for this position include handling case assignments, assessing treatment plans, and informing clients about their mental and behavioral health care options. The clinical care manager’s goal is to ensure that patients receive the best services needed to help them achieve wellness and autonomy.
To ensure success as a clinical care manager, one should have excellent knowledge of case management principles and the ability to act as an advocate for patients in need. Ultimately, a top-notch clinical care manager should be a compassionate, highly organized individual with a passion for children, youth, and their families as well as a passion for mental and behavioral health care.
Position Summary:
- Managing a caseload of 15 clients on a daily basis.
- Handling case assignments by admitting clients to the Gwen’s Girls MH/BH services, reviewing records, and conducting individual, group and/or family therapy orientation.
- Coordinating and providing client care based on an approved treatment plan.
- Facilitating aspects of the treatment plan, such as care coordination and discussions with the family of the client as needed.
- Recording all case information, such as care procedures, appointments, and check-ups.
- Arranging for the intervention of social services, health, or governmental agencies when needed.
- Determining the need for therapeutic, psycho-social, or psychiatric evaluations based on patient progress.
- Interacting with patients to ensure they are informed about their treatment plan, and answering any questions they may have.
- Evaluating patients' progress and writing case reports to determine case closure.
- Building strong relationships with the medical team throughout the case management process.
- Incorporates Sanctuary tools and practices into everyday interactions to create a trauma-informed environment. Upholds the principles of Sanctuary to provide a therapeutic framework and culture.
- Actively participates in all required team meetings, trainings, and supervisory sessions. Meets regularly with supervisor, communicates openly needs, concerns, and ideas; utilizes feedback given in supervisory sessions.
- Performs in accordance with established system-wide competencies and receives satisfactory performance evaluations annually.
- Performs other duties as assigned.
Qualifications
Education: Bachelors or master’s Level professional counselor or social worker in social work, counseling, or a related field with at least 2 years clinical experience.
Licensure/Certification: Licensed Mental Health Professional in the Commonwealth of PA (LCSW, LPC, etc.) preferred.
Minimum Requirements:
- Must have a minimum of 2 years working with special needs/at-risk girls and families, with experience in providing gender-specific, trauma-informed approaches to prevention/intervention services.
- A minimum of 1 years’ experience as a case manager, or a similar role.
- Proficient knowledge of case management principles and behavioral health management.
- Knowledge of inequity issues that impact girls and young women of color.
- Must have the ability to think globally, solve problems and have good decision-making skills, including critical thinking skills.
- Must be able to develop rapport with clients and families, help girls set and achieve goals, and establish appropriate boundaries.
- Must demonstrate an understanding of and respect for the cultural diversity of clients, volunteers, and staff throughout the provision of services.
- Ability to bill.
- Experience working in crisis setting with individuals and families.
- Ability to work flexible hours, including evening and some weekends.
- Strong written and oral communication skills
- PA Driver’s License
Clearances Required: PA Act 33 (Child Abuse) / Act 34 (Criminal History) / Federal Criminal History Record (FBI Fingerprint) / National Sex Offender Registry

Work Environment: Works in an office and after-school community-based setting, with at-risk youth, with limited exposure to adverse environmental conditions.
Physical Requirements: Sitting, standing, kneeling, bending, stooping, reaching over head, speaking, hearing, reading comprehension and the ability to lift and carry 50 pounds.
General Requirements: 1) Indoors in busy office, working in close proximity to co-workers; 2) Frequently required to work at a fast pace; 3) Requires considerable concentration and creativity; 4) Weekend and evening work required; 5) Travel required.
Salary : $41,600 - $52,000
