Shared Living Provider (SLP)

As a Shared Living Provider (SLP), you would be contracted to provide residential supports to a person who experiences an intellectual or developmental disability. The SLP would share their home, while providing supports to include daily living skills, medication administration, transportation, assistance with medical appointments, and activities out in the community.
The SLP would receive a monthly payment from the individual for costs associated with room and board, to include rent, food and utilities. Additionally, Habilitative Opportunities would contract with the SLP for the provision of programmatic services and supports to the individual. Hab Opps pays a competitive contracted rate that is based off of the individual’s authorized budget.
There is an immediate need for an SLP who would be able to provide supports within a wheelchair accessible home.
As an SLP, you will be working with individuals providing assistance in multiple areas, with responsibilities including:
- Assisting with daily activities such as meal preparation and planning
- Assist individuals, as needed, in skill development in areas such as activities of daily living
- Following the appropriate training and supervision, administer medication, as prescribed and in compliance with assigned state regulations and company policies and procedures
- Assist in areas of personal care, hygiene and safety
- Provide habilitation services to each client as directed, according to each client’s individual programs and safety plans.
- Facilitate and encourage community involvement by coordinating and accompanying individuals on outings in the community.
- Provide and maintain accurate, current and thorough electronic documentation through a program called Therap.
Requirements include, but are not limited to:
- Minimum age of 19 years old
- High school diploma or GED required
- Valid Driver’s License and proof of auto insurance
- Complete and successfully pass all background checks, including Federal and state criminal background checks, Nebraska Sex Offender Registry, and Nebraska Adult and Child Protective Services Central Registry
