About the Role:
The Discharge Planner is a member of an innovative care coordination team responsible for connecting individuals and families with substance abuse and mental health needs therapy and care coordination services until linked to long term community providers or consumer needs are resolved. The Discharge Planner is responsible for the provision of community transition services and brief therapy. Patients will be engaged while they are in the hospital and as they are discharging into the community to ensure health care instructions are understood and care transition is seamless and full Collaborative Bridges team services are received.
Qualifications
- Graduate Degree in the Behavioral Health field and a minimum of two years of related behavior management experience
- Communication skills to include interpersonal, teamwork, and collaboration skills
- Conflict resolution and diplomacy skills
- Intermediate level of computer literacy in Microsoft Office, Word, Excel, and Teams
- Must be able to demonstrate excellent interpersonal, verbal, and written communication skills Familiarity and comfortability doing community work in the Westside of Chicago.
- Personal vehicle, valid driver’s license, insurable driving record and vehicle insurance.
Responsibilities:
- Provide trauma-informed brief therapy for individuals with mental health needs to ensure successful community stabilization and transitions of care from hospital settings.
- Complete hospitalization screenings follow-up for consumers prior to, during and after from inpatient psychiatric units and emergency room settings for substance abuse and mental health presentation.
- Ensuring hospital discharge recommendations are understood and completed by the patient.
- Ensure interventions are recovery‐oriented, culturally congruent and developmentally appropriate.
- Administer applicable risk assessment, safety planning, de-escalation interventions, integrating Zero Suicide/Suicide Safer Care principles from admissions to connection with Team Base services.

Crisis intervention skills are utilized daily to address urgent situations, ensuring clients receive timely support. Conflict resolution abilities are crucial for navigating challenging interactions and fostering positive relationships among clients and service providers. Knowledge of mental health and child welfare principles informs the development of comprehensive care plans tailored to individual circumstances. Program development skills enhance the Case Manager's ability to identify gaps in services and advocate for necessary resources within the community.
