As a Home Health Nurse, you will have the opportunity to work with patients and their families in their own homes, providing personalized attention and support. You will be responsible for assessing patient needs, developing and implementing care plans, and coordinating with interdisciplinary teams to ensure seamless care delivery.
Key qualifications for this position include:
- A minimum of six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice
- An Associate's Degree in Nursing or Diploma Certificate in Nursing
- Current NJ State Professional Registered Nurse License and AHA Basic Health Care Life Support HCP Certification
