Overview
The Navigation Coordinator is an advocate, mentor and guide for our patients and their significant others throughout the cancer continuum. This role assists the patient and their significant others by connecting patients directly to services needed throughout the healthcare continuum.
The Center for Discovery and Innovation (CDI) , was established by Hackensack Meridian Health (HMH) as an academic basic and translational research enterprise to rapidly translate science innovation to the clinic to improve outcomes for patients with cancer, infectious diseases, autoimmune disease and other acute and chronic diseases. The CDI has grown rapidly since its origins in 2019, and now has 27 labs, 180 scientists and support personnel. Areas of expertise include genomics, epigenetics, drug resistance, stem cell biology, drug discovery, diagnostics and biomarker development, and pharmacology.
Responsibilities
A day in the life of a Navigation Coordinator at Hackensack Meridian Health includes :
- Facilitates communication between patients and healthcare providers.
- Provides education and educational resources to patients and their families.
- Guides patients through the healthcare continuum and is a strong patient advocate.
- Educates patients on all clinical and supportive services offered at the Cancer Center.
- Meets all new patients and guides them through each area at the Cancer Center during their new patient consultation.
- Facilitates communication for the patient with other disciplines throughout HMH.
- Assists patients with scheduling healthcare appointments, diagnostic and therapeutic services as needed.
- Answers patient phone calls and assists with problem solving and answering questions. If necessary, transfers calls or follows up with the appropriate clinical team.
- Provides expert patient centered care to patients and families.
- Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e. age, culture, language, hearing and / or visually impaired, etc).
- Assists the clinical team to remove barriers for optimal patient care.
- Builds relationships with community members while assessing for social determinants of health needs, where appropriate.
- Other duties and / or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
