Responsibilities :
- Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients.
- Conduct comprehensive assessments of patients' social and emotional needs
- Develop personalized care plans in collaboration with the interdisciplinary team
- Provide counseling and support to patients and families facing end-of-life challenges
- Assist with coordination of care and access to community resources
- Maintain detailed and accurate documentation of patient interactions and progress
- Utilize Homecare Homebase EMR (knowledge of which is a plus)
