Role Overview
Hale Kipa is looking for a Wraparound Facilitator / Care Coordinator to facilitate and coordinate wrapround meetings and services. The Wraparound Facilitator / Care Coordinator is responsible for overseeing the development of the wraparound team and orienting team members to the wraparound process. The Wraparound Facilitator / Care Coordinator coordinates and collaborates with wraparound staff, families and youth, service providers, and community providers to complete the Youth and Family Wrapround plan and Safety / Crisis plan. The Wraparound Facilitator / Care Coordinator also monitors and evaluates progress to ensure the family and youth's goals are being met. This individual also needs to be able to navigate the electronic health record where all client data is stored; and have solid time management and timely documentation skills; as well as being able to communicate effectively both verbally and written.
Qualifications
- Bachelor's degree in a related field such as Social Work, Psychology, Criminal Justice or Human Services, accompanied by experience with youth; OR a minimum of 5 years of experience working with youth.
