POSITION SUMMARY

This position is responsible for the delivery of Intensive in-Home / Intensive Independent Living Skills / Case Management services with children experiencing serious emotional and behavioral disorders.  The position is expected to work within the program and policy directives of the Children’s Services Program of the Waianae Coast Community Mental Health Center, Inc. / Hale Na`au Pono.

REPORTS TO

Administrative: Program Director, Children’s Services.

Clinical: Assigned Therapist for youth served.

Full-Time. Non-Exempt

COMPENSATION

$15.00 hr.

Program Mission

To provide children and youths with serious emotional disturbance a therapeutic family-based alternative to institutionalization as a means of returning them to and/or maintaining them in their home and community.

Target Population

 

 

I. CORE FUNCTIONS

Administrative Duties

  1. Submit charges tags for services provided within one working day.
  2. Receives assignments for the week and/or by the day, from the Program Director/Clinical Director.
  3. Maintains confidentiality of the persons served, and an ethical code of conduct at all times.
  4. Complies with current policies for health and safety procedure at all times.

Family Support/In-Home Services

  1. Participates as a member of a multi-disciplinary team with the persons served, and implements the services that the team prescribes.
  2. Provides one-to-one companionship skills training as scheduled with assigned persons served. Services will be mobile, conducted almost completely in the home or community, and can include the provision of transportation.
  3. Assist when applicable with the admissions process by assisting the persons served with completing the authorizations for information sharing, consents to treat, registration information, orientation and scheduling.
  4. Participates with other service providers, as appropriate, to accomplish joint services planning and to facilitate the provision of services to persons served.
  5. Respects the cultural values and beliefs of persons served.  Provides services with the expected outcome of the interdisciplinary team and the persons served.
  6. Provides face-t-face contact, and document applicable information according to policy in the progress notes records of the persons served within one working day. The date, time, duration and service codes indicated on the progress notes will correspond with the master treatment plan.
  7. Maintaining case records of persons served according to policy, and making face-to-face contact according to treatment plan at least once per week.
  8. Completes case management monthlies for assigned persons served according to procedure every month.
  9. Maintains records of assigned persons served to comply with quality assurance reviews.
  10. Serves on-call 24 hours, 7 days a week and provides crisis intervention and stabilization services as assigned.
  11. May assist more experienced staff members conducting psycho-educational groups.
  12. Participates in the Treatment Team meetings with the child and family and other team members.
  13. Supports the implementation of assigned tasks defined in the Mental Health Treatment Plan under the supervision and direction of the assigned therapist.  Acts as a consultant and coach with assigned families, working in the home, at school or where ever the need arise.
  14. Provides as much time with youth and the family as requested.
  15. Intervening with the family using strength base approaches, strategies for parenting home/behavior management skills are developed which are needed for the family to handle on-going.
  16. Adhere to CASSP Principles.

II. OTHER FUNCTIONS

  1. Performs other related duties as assigned.

III. QUALIFICATIONS

Knowledge / Abilities

  1. Knowledge of the culture and lifestyle of the Community.
  2. Able to interpret problems within a cultural context
  3. Ability to apply cultural concepts in interventions.
  4. Personal qualities or warmth, acceptance, sound judgement, emotional stability, and reliability.
  5. A desire to work with children experiencing serious emotional disorders.

Education/Experience

 

 

Physical Requirements

  1. Bachelor’s degree (BSW) and 2 years’ experience: or a high school diploma or GED and at least 4 years of case management experience, preferably with children experiencing serious emotional disorders.
  2. Must be physically able to perform job duties with or without reasonable accommodation.  Any physical condition that could cause the applicant to be a hazard to himself/herself or to others, will disqualify him/her for appointment.

IV. OTHER REQUIREMENTS

 

  1. Valid driver's license and clean driving record within the past three years and ready access to an insured vehicle.
  2. Availability to work flexible hours, including nights, weekends and holidays as needed.
  3. Consent to background checks and drug testing as per organizational policy.
  4. Consents and submits to drug testing.
  5. Evidence of full vaccination against COVID-19 and compliance with health and safety protocols.
