Halifax Health - Hospice is seeking a Bereavement Counselor.
Summary
This individual is responsible for assessing and assisting with social and emotional factors related to the bereavement process as it relates to assigned pediatric, adolescent, adult or geriatric Hospice patients and his/her family or caregiver. This individual will participate in staff orientation, ongoing education and support. Coordinates and facilitates bereavement groups in Volusia/Flagler area.
Job Qualifications
- Master's Degree in Social Work, Counseling or other related field required. Licensure preferred.
- Current licensure under Chapter 491 of the Florida State Code preferred or working toward said license.
- Counseling/Social Work background; experience in bereavement counseling preferred.
- Working knowledge of grief and bereavement, individual group counseling, interest/experience in gerontology.
- Ability to cope with high stress levels.
Job Duties and Responsibilities
- Provide bereavement services to an assigned pediatric, adolescent, adult or geriatric Hospice patient and his/her family or caregiver as requested.
- Assess the social and emotional factors related to family members/survivors/primary caregivers coping with a loss.
- Provide bereavement supportive counseling services to assist those survivors in the resolution of grief reactions. This provided on an individual basis.
- Participates at meetings and acts as a consultant to the Team Members.
- Maintains clinical records including but not limited to bereavement assessment, follow-up evaluation and progress notes.
- Educate community in the needs of the bereaved.
- Participate in the selection, training and continuing education of the Bereavement Volunteers.
- Coordinate Bereavement Team Meetings.
- Participate in support network/mechanism for Hospice staff.
- Performs other duties, as assigned.
