Registered Nurse -Oncology Patient Navigator
Assumes responsibility and accountability for the management of post-acute resources through interdisciplinary collaboration. Works closely with care management, physicians and post-acute providers in various specialty areas.
- Bachelor’s degree in Social Work or a licensure as a Registered Nurse/Licensed Practical Nurse.
- Minimum of three years’ experience in the medical field.
- Self-directed and able to organize and manage multiple tasks/projects simultaneously.
- Extensive understanding of patient care.
- Proficient in review of medical records and other patient care information.
- Demonstrated skills in verbal and written communication.
- Provide emotional support and counseling related to the clinical situation.
- Partner with designated physicians to develop and maintain protocols.
- Involved with the development and enhancement processes with the aim to improve the clinical experience for referred patients or physicians.
- Communicates with referring physicians’ offices as required by the patient and physician.
- Collaborate with marketing and outreach departments to educate referring physicians and facilities on services.
- Develops strong working relationships with the post-acute care team, including those that facilitate patient care across the care continuum.
- Develops monthly quality/risk meetings with SNFs and medical staff to review metrics, root cause on return to hospitals, to further partnership of acute care and post.
- Works closely with the post-acute providers as well as those we partner with to facilitate care for our patients in the post-acute setting ensuring patients care transitions from acute care to post-acute care are seamless.
- Designs and coordinates continuing care communications programs to inform preferred provider organizations, caregivers including physician, nursing and case management, as well as patients and their families.
