About the job : We are looking for a physician who is licensed to practice medicine orosteopathy in Massachusetts, and has the knowledge, skills, and interest in themedical, psychosocial, and spiritual aspects of hospice care. Board certification in Hospice and Palliativemedicine is preferred. The Medical Director assumesoverall responsibility for the medical component of the Hospice's patient careprogram and is a liaison with the medical community. The hospice Medical Director, alongwith the patient s attending physician, is responsible for meeting thepatient s needs for palliation and management of the terminal illness andrelated conditions. The Medical Director has overallresponsibility for the medical component of the Hospice patient care programincluding, but not limited to, attending interdisciplinary team meetings andserving as the physician member of the IDT, assisting in developing plan ofcare for patient / family, and is a resource to other team members, with respectto medical management of patient s plan of care. The Medical Director certifies(in conjunction with the attending physician) that the patient is terminallyill and completes all Certificate of Terminal Illness (COTI) documentation. The Medical Director acts as medical liaisonwith physicians in the Hallmark Health System and the community. Activeparticipation in Hospice QAPI (Quality Assurance and Performance Improvement)is required. The Medical Director performs an active role in promoting Hospiceand Palliative Care, including participation in program development specific toproviding education to members of the medical community regarding palliativeand end of life care.
