Job Description:
As our Business Development Manager, you will play a crucial role in expanding our referral network and ensuring seamless transitions for our clients. The ideal candidate will have existing referral relationships and can create strong, new connections with the community we serve.
Cultivate Referral Sources:
- Work closely with hospital discharge planners, social workers, case managers, and physicians to generate referrals.
- Provide educational presentations to staff and community members.
- Actively participate in networking events within assigned territories.
- Travel frequently to scheduled meetings and develop referral sources
Manage Admission Coordination:
- Oversee the referral-to-admission process, ensuring efficient documentation and processing.
Ensure Continuity of Care:
- Facilitate smooth communication between physicians, community resources, and our home care teams.
Conduct In-Home Assessments and Care Planning:
- Provide initial in-home assessments for prospective and new clients.
- Document assessment notes and develop comprehensive care plans.
Lead Management and Start of Care:
- Manage leads from referral sources and ensure efficient start of care for new clients.
- Coordinate with team in providing detailed information for successful staffing and client care.
Community Education:
- Educate patients, families, community and other referral sources on programs and services offered.
Qualifications:
- Proven track record in home health sales and business development.
- An existing book of business IS REQUIRED.
- Existing relationships with physicians, case managers, discharge planners, and other healthcare professionals.
- Strong communication, negotiation, and presentation skills.
- Self-starter with the ability to work independently.
- Goal-oriented and driven to meet quarterly and annual targets.
