Essential Responsibilities
- Provide comprehensive case management, including intake and referral services to residents in need.
- Document all activities and prepare required reports to ensure accurate tracking and accountability.
- Forge and maintain relationships with community agencies and service providers to ensure residents have access to flexible, creative, and individualized services.
- Maintain an up-to-date directory of service providers for use by project staff, residents, and their families.
- Monitor and assess the ongoing delivery of services from community agencies, keeping both case management and provider agencies informed of residents’ progress.
- Oversee the provision of supportive services as needed, ensuring they align with residents' specific requirements.
- Perform other duties as assigned to support the effective operation of the program
- Educate residents about available services, application procedures, and client rights, advocating on their behalf as needed and referring them to appropriate community service providers.
- Facilitate the development of informal social support networks among residents, their families, and friends.
- High School diploma or GED equivalent.
- Ability to pass the American Association of Service Coordinator Program within the first 24 months of employment.
- Effective communication skills, both verbal and written.
- Thorough understanding of and adherence to all safety rules, regulations, policies, and procedures, including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to Know, SDS, ADA, HUD Policy and Procedures, and Fair Housing laws and regulations.
- Strong organizational and multitasking abilities to manage diverse responsibilities effectively.
- Proficiency in utilizing various software and tools for documentation and reporting.
- Exceptional interpersonal skills to build and maintain productive relationships with residents, service providers, and community organizations.
- Ability to handle sensitive situations with empathy, discretion, and professionalism.
- Knowledge of community resources and the ability to leverage them to meet residents' needs effectively.
- Capacity to stay informed about changes in policies, regulations, and best practices relevant to the role.
