Job Summary
Under the supervision of the Access to Care Coordinator, the Access to Care Case Manager (ACCM) provides screening, assessment, and case management services for clients in the access to care process. The ACCM coordinates care with intra-agency staff, county workers, school personnel, referring agencies and the community at large. Additionally, they complete and submit reports required by the agency and the Department of Mental Health (DMH).
Essential Functions
- Maintain effective working relationship with and coordinate care with intra-agency staff, county workers, school personnel, referring agencies, and the community at large.
- In collaboration with their supervisor, be able to identify and report (e.g., potential Suspected Child Abuse Reports, other crisis situations)
- Complete and submit reports required by DMH (e.g., school based monthly tracking, school site quarterly report) and the agency (e.g., Initial request/referral log, Case Record Review) in accordance with DMH and agency workflow and guidelines
- Conduct screenings for referrals, determine the appropriate level of care for the client.
- Maintain/update the referral list daily with client data entry
- Respond to referrals within 24 hours of inquiry
- Inform staff of case assignments via email and/or directly book the appointment for the therapist.
- Notify medication support services regarding immediate referrals, gathering all pertinent information for services to commence.
- Works to promote a trauma-informed work environment.
- Actively participate in the quality improvement project
The ACCM will take on special projects or additional duties as assigned by the Access to Care Coordinator.
Competency Requirements
-Ability to speak, read, write, and understand English
-Ability to work effectively as a member of the treatment team
-Ability to promptly and appropriately respond to a crisis situation
-Must have excellent oral and written communication skills
-Must demonstrate attention to detail
-Must have excellent customer service skills and phone manner
-Bilingual (English/Spanish) required
Education and Experience
-Bachelor’s degree in related field strongly preferred
-Previous experience working in a social services agency in a similar capacity preferred
Certifications/Licenses
-Valid CA driver’s license
Additional Requirements
-Tuberculosis clearance
-Department of Justice/Fingerprint clearance
-Satisfactory completion of health screening
-Proof of legal eligibility to work in the United States
-Satisfactory driving record

Physical Demands
While performing the duties of the Access to Care Case Manager, the employee is frequently required to sit, stand and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to walk, bend, drive and lift objects heavier than ten pounds. The employee is rarely required to run.
Work Environment
The work environment is primarily indoors. The noise level varies from quiet to moderate.
