As an Eligibility Specialist, your day revolves around performing the Children’s Ohio Eligibility Determination Instrument (COEDI) or the Ohio Eligibility Determination Instrument (OEDI) assessment tools. You will connect to families to schedule and conduct these assessments. After conducting thorough assessments, you'll enter the data into both local and state databases and process eligibility packets. Once results are finalized, you’ll notify families of the outcomes, providing them with clarity and guidance. Throughout the day, you’ll also offer community resources and referrals for those seeking waiver or community-based services. To ensure continuous improvement, you’ll track data and trends related to outcomes and share key insights with your supervisor, helping to enhance service delivery across the organization.

- After the initial training period, hybrid work-from-home options are available. Hours will be fixed with exact schedule to be determined upon hire.
- One week of paid vacation and 15 sick days immediately allocated upon hire. 3 personal days provided per year, prorated with employee start date. Starting vacation accrual of 3 weeks per year. Sick time accrued at 15 days per year after first year of employment with an unlimited accrual.
- Starting salary range, determined by formula accounting for years of experience: $45,240.00 (base rate) to $56,550.00 (10 years of experience performing this same job or job with primarily the same duties.)

Key Responsibilities
- Eligibility Determination & Assessment (60%)
- Conduct eligibility determinations for services and nursing home placements using state-approved instruments and other assessment tools.
- Utilize independent judgment to make subjective determinations based on assessment criteria, observations, and professional discretion.
- Provide recommendations for services based on assessments and tools, and meet with agency social workers and supervisors to present findings as needed.
- Community Resource Coordination (25%)
- Assist individuals and families in identifying and selecting appropriate services and support from external community resources outside of the agency.
- Documentation & Reporting (15%)
- Maintain accurate progress notes and prepare reports as required, ensuring confidentiality is upheld.
- Perform other administrative tasks as assigned.
- Bachelor's degree in social work, psychology, special education, human services or related field of study
- One year of full-time paid work experience in programs or services for individuals with developmental disabilities
- Must be able to pass criminal background checks and various abuser registry checks
