What are the responsibilities and job description for the Population Health Specialist position at Hamilton County Public Health?
Hamilton County Public Health is accepting applications for a Population Health Specialist in the Department of Community Health Services. Requirements include: BS in health education, public health, social work, or related field, one (1) year experience which indicates a thorough knowledge of population health/health promotion and health education principles. Master’s Degree preferred. CHES®/MCHES®preferred. Candidate with knowledge of population health/community health education practices, and policy, system and environmental change strategies preferred. Experience with authentic community engagement and ability communicating with diverse audiences preferred. Must possess a valid driver’s license issued by the state of residency. Some evening and weekend hours required.
Job Duties include but are not limited to:
- Plan, coordinate and implement population health/health promotion and education strategies, initiatives, and training
- Establish and maintain working relationships with various stakeholders and communities.
- Facilitate county-wide mobilization for policy, systems and environmental change related to health, safety, and vitality.
- Evaluate project outcomes.
- Identify best practice recommendations for policy and system changes that can be implemented at the community level (Village, Township, City) that impact the overall health, safety, and vitality of the community.
- Translate public health best practices for populations served.
- Serve as a resource for population health/health promotion and education for communities within Hamilton County.
- Report population health/health promotion and education activities and outcomes to all key stakeholders as defined.
- Provide support for public health emergencies.
