Position Summary
Hamilton-Madison House’s Supported Housing program provides permanent housing options for individuals with Serious and Persistent Mental Illness (SPMI), enabling them to live independently within the community. The case manager will deliver high quality support services to tenants participating in the Supported Housing program
Responsibilities
- Provide comprehensive case management services to program participants
- Conduct home visits at least once a week to monitor and support clients
- Collaborate with clients to de3velop and implement individual Service Plans (ISP) aimed at enhancing independent living skills, self-sufficiency, income, and residential stability.
- Identify potential crisis situations and provide appropriate interventions.
- Assist participants in accessing necessary supportive services, including mental health care, primary healthcare, and substance abuse support.
- Address participants/ needs by obtaining forms, completing paperwork, contacting referrals, writing correspondence, making phone calls, and resolving disputes.
- Provide referrals and information on available resources, conducting research as needed.
- Provide transportation to appropriate community-based resources
- Provide consultation and support outside of regular hours as needed.
- Work closely with state psychiatric staff to facilitate smooth transitions from centers to supported housing units.
- Perform duties as assigned by the supervisor or Assistant Executive Director of Behavioral Health and/or Supported Housing Program Manager
Minimum Qualifications
- BA in social work, mental health or psychology or related field preferred
- Bilingual English/Chinese preferred
- Good driving record required
- Excellent interpersonal skills
- Good organizational skills
Hours: Monday to Friday, 9:00AM to 5:00PM, 37.5 hours per week
Compensation Range: $25 - $26 per hour depending on qualifications and experience
