About the Role

Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. • Maintaining records of outreach contacts and activities.
  • Serve as the initial contact for patients seeking behavioral health services, work with supervisor to conduct pre-admission screenings and deliver health related social needs (HRSN) with patients & families.
