Description:
Hamilton Memorial Hospital District has an immediate opening for a full-time Unit Clerk at the Senior Enrichment Center. At HMHD our mission is to enhance the delivery of healthcare to our community and the surrounding area by providing accessible quality healthcare services.
Pay Scale: $15.30-$22.03 per hour
Job Summary:
The Unit Clerk will perform secretarial/clerical duties in the geropsychiatric office. They will function as the focal support and communication source in the office. The Unit Clerk will assess patients, ensure the care of patients, provide patient/family education regarding area resources and assistance for patient support and discharge planning, and adhere to patient rights and organization ethics. They will provide a continuum of care, manage information, improve organizational performance, and maintain the environment of care.
Essential Functions:
1. Is a role model in regard to displaying a professional manner, as well as working relationship on the unit and within the hospital.
2. Communicates complaints and/or suggestions through appropriate channels.
3. Fulfills responsibilities in a reliable and dependable manner and uses time effectively.
4. Recognizes learning needs and shows motivation to meet these.
5. Demonstrates respect for patient rights by maintaining patient privacy, dignity, safety, confidentiality, and right to refuse or consent to treatment.
6. Uses professional nursing principles, procedures and techniques in the care of psychiatric patients. Communicates calmly with patients, family and visitors in situations that require tact and diplomacy.
7. Has ability to communicate needs of patients to the treatment team.
Position Responsibilities:
- Assesses and reassesses the noon clinical needs of the patient.
- Records proper documentation of observed needs.
- Has ability to communicate needs of patients to the treatment team.
- Provides input into efficient discharge planning throughout treatment process.
- Completes documentation in the patient charts according to program procedures.
- Active member of the Interdisciplinary Team.
- Answers unit phone in a courteous fashion.
- Communicates pertinent patient care information and general environmental information to nurse and other staff.
- Observes hospital and unit policy in assisting and directing patients, staff, and visitors.
- Functions as a support to program nurse during emergency situations.
- Provides ongoing resource education to patient and family as needed.
- Provides resources and support for patient before, during and after treatment of patient.
- Assists and coordinates patient transportation as applicable.
- Actively involved in interacting and observing the patient performance as it pertains to treatment goals and issues.
- Communicates observations and interactions to supervisor and Treatment Team.
- Provides feedback to supervisor on ongoing basis in regard to unit performance and service concerns, improvements, changes, etc.
- Identifies areas needing improvement and utilizes the facility performance improvement process. Active participation in department processes is required.
- Ensures facility is safe and reports deficiencies to supervisor.
- Maintains adequate quantity of forms, materials, supplies and equipment inventory on unit.
- Maintains assignment boards, bulletin boards, and other communication sources.
- Maintains a clean, safe and functional work environment on an on-going basis.
- Coordinates the ordering and delivery of patient meals and snacks on the unit.
- Completes required competencies per program regulations.
- Attends in-services, seminars, etc. to promote learning.
- Ensures confidentiality of information encountered.
- Initiates, assembles, and maintains charts for patients on the assigned unit.
- Completes billing forms.
- Maintains the patient chart in an appropriate, function order, as prescribed by the hospital and unit policy.
- Assists patients with personal care needs (such as one-assist transfers, bathroom, etc.) as needed or requested
- Monitors location of chart, has chart ready when patient goes to another department and transfers chart to Medical Records Department upon discharge of patient.
- Provides transportation to and from program for patients if circumstances prevent our contracted agencies from doing so, per transportation policy guidelines.
- Performs routine housekeeping duties (dusting, vacuuming, taking out trash, cleaning restrooms, etc.) on a daily basis.
