Position Summary:
The Resident Care Coordinator assists the Executive Director in the daily routine and operation of the department. Has basic knowledge and understanding of dementia, mental illness and general medical problems encountered by the elderly. Collaborates with the Executive Director with daily and routine duties including, but not limited to, the following responsibilities listed below.
Essential Duties and Responsibilities:

- Assists with new move-ins and transfers of Independent Living residents including but not limited to generating resident
- Assures new residents are properly
- Educates Residents on third party providers and assist with coordination of needed health
- Observe Residents for change in condition and report any changes immediately to Executive Provide timely interventions to change of condition and share this with Executive Director and family as indicated.
- Daily check in for all Independent Living residents who request service with Great Call
- Execute weekly and EOM occupancy reports and additional reports when
- Follow Independent Living residents when they go to hospital or rehab to ensure return at proper
- Assures residents in need get referred to in house therapy, private duty, medical services and hospice or provider of
- Support fall prevention programs for Independent Residents in need. This includes proactive intervention use, monitoring, safety in the apartments and
- Assist the Executive Director in planning, organizing, and supervising the Department to meet the needs of the residents and
- Provide training to department Associates to recognize and report changes in
- Meet defined metrics set-forth by the
- Maintains sales collateral, occupancy reports and Facebook and Website
- Participate in all staff activities designed to develop teamwork, unity and promote

Supervisory Responsibilities:
- The position falls under the direct supervision of Executive Director and is expected to perform independently and exercise good judgment. The position does not supervise, direct, or review the work of the or other associates.

Educational Requirements and Experience:
- High school diploma or equivalent (GED)
- At least 2 years of related experience, preferably in a senior living or equivalent
- Nursing Assistant Training Program certificate (approved by the State Board of Nursing) preferred.
- Certified Nursing Assistant (CNA) certification
- CPR and First Aid certification
Knowledge, Skills. and Abilities:
- Ability to read and write, follow written and oral instructions, communicate effectively in English and experience in excel, social media and other general administrative Ability to work with the elderly in a courteous and friendly manner, demonstrating patience and compassion towards the elderly. Ability to perform duties with consideration for residents' rights at all times and demonstrate integrity and confidentiality in the care of residents and in handling their health information.
Technical Skills:
- Knowledge of medical terminology, medical records management, medical supplies, and equipment preferred. Ability to perform basic adult CPR and basic adult first aid procedures
Physical/Sensory Demands:
- On a daily basis, throughout the workday: Must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching and kneeling, reaching over the head, grasping with both hands, and fine manipulation. Must physically be able to lift up to 50 pounds unassisted. Must physically be able to carry, transfer, push and pull, and reposition residents with assistance or using appropriate equipment. Must have normal eyesight or use corrective Must have three-dimensional vision and ability to judge distances and spatial relationships so as to see object where and as they actually are. Must be able to hear and distinguish between normal tones and be able to perceive the nature of sounds. Must be able to exchange ideas by means of the spoken word as well as engage in activities to convey detailed or spoken instructions to other workers accurately, loudly, and/or quickly.
Environmental Adaptability and Safety:
- Works in well-lighted, well-ventilated buildings. Must be able to tolerate occasional exposure to dust, fumes, odors, water, etc., as well as some noise. When outside, must be able to tolerate exposure to weather, fluctuations in temperature (hot, cold), wet and/or humid conditions. Must be willing to work, when necessary, beyond normal working hours and on weekends, as well as in other positions as necessary.
