As a full-time case manager at the Hampton-Newport News Community Services Board, you will play a vital role in the Regional Reinvestment Project to optimize mental health care by reducing reliance on state psychiatric facilities. What we're looking for in a Case Manager-Reinvestment

To excel as a full-time case manager at the Hampton-Newport News Community Services Board, a solid foundation in Human Services is essential. Candidates should possess a Bachelor's degree in fields such as Social Work, Psychology, Sociology, or Counseling, paired with a minimum of one year of experience in case management services. Proficiency in relevant software and tools for case tracking and documentation is crucial for effectively managing client cases.

The ability to establish rapport with clients and navigate community resources is paramount for successful discharge planning and ongoing support. Additionally, a valid Driver's License is required, as transporting clients may be necessary to facilitate access to essential services. Strong communication skills, empathy, and a commitment to client-centered care will further enhance your effectiveness in this vital role.

The knowledge and skills required for the position are:

• Bachelor's degree in Human Services such as Social Work Psychology Sociology or Counseling
• One (1) year experience in case management services.
• Valid Driver's License. Transports clients as necessary.
