Your role as a Support Coordinator - Child and Adolescent Developmental Services
As a Support Coordinator for the Child and Adolescent Developmental Services program at Hampton-Newport News Community Services Board, you will play a vital role in ensuring continuity of care for individuals with intellectual and developmental disabilities. In this position, you will assist clients with their basic physical, financial, social, and personal needs, while developing person-centered plans tailored to each individual. Your responsibilities will include linking clients with service providers, monitoring progress, and advocating for their needs both within and outside the system.
By serving as a trusted case manager, you will guide families through the complex service landscape, fostering informed choices about the supports available to them. Your efforts will focus on enhancing community integration and addressing a wide array of needs-medical, educational, vocational, and recreational-while ensuring they achieve desired outcomes aligned with discharge protocols.

To excel as a Support Coordinator in the Child and Adolescent Developmental Services program at Hampton-Newport News Community Services Board, candidates must possess a bachelor's degree in human services, such as Social Work, Psychology, or Counseling, complemented by at least one year of experience in support coordination services for individuals with intellectual and developmental disabilities. Strong interpersonal skills are essential, as you will be the primary contact for clients and their families, building trusting relationships and fostering open communication. Proficient use of relevant software and tools is required to effectively create and manage person-centered plans, monitor progress, and document services.
Additionally, a valid Virginia Driver's License is necessary to facilitate transportation needs for clients. A deep understanding of advocacy, crisis intervention, and community resources will further ensure your success in this vital role, allowing you to effectively navigate the service system and meet individual needs comprehensively.
Knowledge and skills required for the position are:
- Bachelor's degree in human services such as Social Work, Psychology or Counseling with one (1) year experience in support coordination services with the assigned population.
- Valid Virginia Driver's License.
