Responsibilities:
- Provide direct patient care in a long-term care, nursing home, and rehabilitation center setting.
- Administer medications and treatments as prescribed by physicians.
- Monitor and assess patient conditions, reporting any changes to the healthcare team.
- Assist with patient admissions, discharges, and transfers.
- Document patient information accurately in electronic medical records (EMR).
- Collaborate with healthcare professionals to ensure quality patient care and outcomes.
- Maintain and uphold patient confidentiality and adhere to HIPAA regulations.
- Participate in ongoing training and professional development opportunities.
- Supervise and support nursing staff, ensuring adherence to best practices and protocols.
- Address patient and family concerns, providing information and support as needed.
- Manage patient care plans, ensuring they are updated and followed.
Qualifications:
- Current and valid LPN certification (required). RN license is a plus.
- Proficiency in general patient care.
- Experience with PointClickCare a plus.
- Ability to work effectively with diverse patient populations, particularly in geriatrics.
- Ability to work flexible hours, including day and night shifts, holidays, and overtime.
- Willingness to work a 3x12 hour shift schedule with rotating weekends.
Salary : $32 - $34
