Disability Case Management Coordinator
Position Summary
The primary function of this position is to complete and review FMLA documents, long and short-term disability questionnaires, and the ability to author disability correspondence for a fast-paced medical practice. There will be occasions where back-up for both positions will be necessary. This position will include cross training for both departments which are disability and worker’s compensation.
Education and Experience
High school diploma. Some higher education preferred. Minimum of two years’ experience in the medical and/or disability field.

Ability to communicate effectively verbally
Ability to interact with team members, patients, and employers in a variety of settings.
Ability to prioritize work with minimal supervision to independently carry out the duties of this position.
MUST be proficient with time management
Willingness and ability to be trained in the other positions in the department which would include Workers’ Compensation
Knowledge of Microsoft Office and Medical EMR systems.
Responsibilities
Complete, review, edit/update and forward all FMLA forms, short and long term disability forms, questionnaires for all medical teams.
When necessary, back up the Workers’ Compensation Coordinator.
Follow up with telephone calls and keep all patients informed of the status of their disability and/or FMLA forms.
Inform the Director of any issues regarding patients, employees and/or questions regarding forms, and job duties.
PHYSICAL DEMANDS
Normal demands associated with an office environment. Ability to communicate with individuals by telephone, email, and face to face. Ability to lift up to 30 pounds.
