Hancock Park Rehabilitation and Nursing Center , an award-winning skilled nursing and rehabilitation facility located in Quincy, MA, is seeking a full-time  Director of Social Services   to join our fantastic team for our Short-Term and Long-Term residents. Requirements for Director of Social Services :

  • Must possess a Bachelors degree in Social Work, at minimum.  MSW candidates preferred
  • Must hold an active, unencumbered license as a Social Worker in the state of Massachusetts

    • Must be an LSW at minimum, LCSW / LICSW preferred.

  • Nursing home / post-acute experience, preferred
  • Job Duties of Director of Social Services :
  • Compiles social history data
  • Participates in assessments, development and evolution of residents care plan
  • Responsible for planning and leadership interdisciplinary care planning weekly meetings
  • Documents timely progress notes according to state and federal regulations
  • Coordinates the discharge planning for residents
  • Able to interpret and implement established social service policies, procedures and regulations
  • Coordinates services of community agencies for continued delivery of care upon discharge
